We work with families, physicians, and other healthcare providers to create a personalized plan of care that allows patients to return from the hospital earlier, remain in their homes longer and help prevent return hospitalizations.
In addition, we offer the ability to continue health care access in the comfort of your own home, as well as personal advanced health care planning.
SKILLED NURSING
Evaluate personal health and home environment. Provide services such as wound care, IV care and medication management.
Full-service therapy for patients/clients in the comfort of their home, including:
PHYSICAL THERAPY
Provide individual home exercise plan. Encourage safety independence and assess/treat risk of falling.
OCCUPATIONAL THERAPY
Help improve strength, balance and safety. Recommend solutions to increase independence at home.
SPEECH THERAPY
Evaluate speech limitations and disorders. Improve memory and reduce the risk of aspiration and choking.
OTHER SERVICES
Manage chronic disease, recovery from illness, injury, or surgery. Assess needs and identify limitations and/or safety concerns. Connect patients and families to necessary resources and support in the community.
